Through our partnership with Summa Sports Medicine, we are pleased to offer sports physicals at CVCA this year. We will be offering exams on Thursday, May 30th starting at 6pm. Registration is required and can be completed using the link below. The cost is again $20 per physical and the physical will be valid for the entire 2024-2025 school year. Time slots have been assigned based on the first letter of your last name to allow for a quicker experience. Please plan to bring a copy of your OHSAA PPE Physical form found in your FinalForms account. 2024-25 Sports Registration will open on May 1st.
